Deliver primary care to residents on-site at nursing homes, assisted living and senior living communities across Florida. You will see patients in their own rooms on a set schedule, working alongside facility nursing staff.
Responsibilities
- Conduct wellness visits, acute visits and follow-up care at the bedside
- Perform medication reviews and manage ongoing prescriptions
- Order and coordinate labs and diagnostics
- Make specialist referrals where needed
- Document findings and care plans into facility records after every visit
- Communicate clearly with facility staff and families
Requirements
- Active Florida Nurse Practitioner license in good standing
- Experience in geriatrics, long-term care or primary care preferred
- Comfortable working independently on-site with strong clinical judgment
- Reliable transportation and willingness to travel between facilities
- Strong documentation habits and communication skills
